This study was performed to investigate the condylar bony change in radiographic image, focusing the relationship bet ween the bony change by observation and the radius of curvature of the condyle by measurement. Ninety patients with internal derangement of the temporomandibular joint and thirty dental students without symptomatic temporomandibular joint disorders were selected for the study, and patients were classified into three groups, namely, disc displacement with reduction group(group 1), closed lock group of less than 35mm opening(group 2), and closed lock group of 35mm or over opening(group 3), respectively.
Panoramic and transcranial radiographs were taken and the condylar bony change by visible observation was assessed, first, according to the criteria based on Boering's report, such as 0 : no bony change, 1 : flattening, 2 : erosion. 3 : shortening and/or dimensional shrinkage, and second, according to the location of condylar bony change, that was, 0 : not visible. 1 : bony change in the anterior or posterior articular surface. 2 : bony change both of the anterior and posterior articular surface. The condylar shape was also assessed by measuring the radius of curvature of condyle in anterior, superior, and posterior articular surface, respectively, and in the whole condyle.
Data were processed with SPSS windows® (ver. 10.0) and the results obtained were as follows:
1. There were not significant difference in the degree of condylar bony change between the affected and non affected side for the all observed case by the Boering's, but the degree of bony change were higher in the condyle showing both in the anterior and posterior surface resorption.
2. The radius of whole curvature of condyle showed a tendency of decrease as the degree of bony change increased, and in the condyle showing both in the anterior and posterior resorption, but the radius of anterior or posterior curvature of condyle showing resorption of the articular surface tended to be larger than the condyle showing no bony change.
3. The radius of superior curvature showed very significantly negative correlation with the number of articular surface showing resorption, that is, the more the resorption site were, the less the radius of curvature was.
4. Patients with closed lock(group 2 and 3) showed a tendency of lower condylar and ramus height, and more condylar bony change than the patients with disc displacement with reduction(group 1) in affected condyle.
