Immunization is known as one of most cost effective method to prevent a disease. Therefore many countries provide a financial support to increase vaccination rate.
Korea has the financial aids program for national immunization program also. Nowadays, financial aids programs expand its coverage. In 2014, financial aids programs initiated the support for the streptococcus pneumoniae vaccination. Also the vaccination for hepatitis A will be supported in 2015.
Though the coverage has expand, the process of decision is not discussed enough. Especially, priority for coverage for vaccination is in question. To make a information for decision making, we tried to measure the burden of vaccination related diseases-influenza, hepatitis A, streptococcus pneumoniae, rota virus, human papilloma virus. The burden of diseases were measured by prevalence, mortality and economic burden. Also the expert opinion is acquired, and the budget for coverage is also estimated. To estimate the burden of disease, the claims data of national health insurance was mainly used.
As a result, the order of prevalence was estimated as influenza-human papilloma virus-rota virus-hepatitis A-streptococcus pneumoniae. In case of mortality, the order was human papilloma virus-influenza-streptococcus pneumoniae-hepatitis A-rota virus. The economic burden was estimated as same order. But in the case of budget, streptococcus pneumoniae needed number one budget.
Considering, the burden of disease and the efficacy of vaccination, we recommend the priority of coverage expansion should be influenza-human papilloma virus-rota virus. More importantly, the process of decision making for coverage should be discussed before the decision to avoid unnecessary social problem.
