Backgrounds and Objectives : Transcranial Doppler ultarsonography(TCD) is a useful tool for assessment of the intracranial circulation in patients with cerebral arteriovenous malformation(AVM). This study performed to define the factors related with blood flow velocity of TCD in AVM patients and to understand the hemodynamic change in AVM. Methods : AVM patients who underwent TCD and cerebral angiography prior to treatment were recruited. The mean flow velocities(MFV) were recorded in bilateral anterior(ACA), middle(MCA) and posterior cerebral arteries(PCA), and in basilar artery(BA). Pathologic MFV of each artery was defined as above mean±2SD of MFV calculated from normal control group. Variables related with pathologic MFV were determined as feeder, AVM size(≤2.5 cm and >2.5 cm), number of feeders, location deep and superficial) and hemorrhage. Frequency of pathologic MFV of feeders according to variables was analyzed. Statistical analyses were used by student t-test and Chi squares. Results : Seventeen patients, 12 male and 5 female, were recruited. The mean age was 31.2±12.0 years. Control group consisted 9 males and 6 females and their mean age was 33.9±7.2 years. There were 10 small size(≤2.5 cm) AVMs and 7 medium and large AVMs. Superficial and deep AVMs were 12 and 5, respectively. AVMs with one feeder were 9 and those with two or more feeders were 8. Ruptured AVMs were 9 cases. Among 23 feeders, 15(65%) revealed pathologic MFV. Among 86 nonfeeders, 19 vessels(22%) were pathologic MFV. By frequency analysis, feeder and size more than 2.5 cm were related with pathologic MFV(p<0.001, p<0.05, respectively). But number of feeders, location and hemorrhage were not statistically related with pathologic MFV. Conclusions : This study revealed that feeder and size of AVM were related with increased blood flow velocity in AVM. To define the relationship between other variables and blood flow velocity, further investigation with large number of AVM patients is required. Korean Journal of Stroke 1999;1(2): 182~188
