Kikuchi병은 경부 림프절 종대와 발열을 특징으로 하는 드문 질환으로 특별한 치료 없이 자연 치유되는 특징을 가지고 있으며 림프절의 조직학적 절제 생검을 통해 진단된다. 특히 Kikuchi병이 폐결절로 오인된 경우는 아직까지 보고된 바가 없었다. 저자들은 45세 남자가 근육통, 피로, 발열을 주소로 내원하여 시행한 흉부 전산화 단층촬영에서 좌상엽 폐소설 분절의 결절이 이전과 비교해 커진 소견을 보여 조직검사를 시행하였고, 조직검사에서 괴사성 조직구 림프절염 소견이 관찰되어 Kikuchi병을 진단하였다. 소염진통제 및 대증적 치료로 증상은 호전되었고 종물의 위치가 기존에 알려져있던 경부림프절, 액와부, 상쇄골, 서혜부, 장간막 림프절 등이 아닌 폐내 림프절에서 발생하여 폐결절의 형태로 오인되었던 Kikuchi병 1예를 경험하였기에 문헌 고찰과 함께 증례를 보고하는 바이다.
