외상 또는 응고 장애 등이 동반되지 않는 상태에서 자발적인 피막하 혈종의 발생은 드문 경우이다. 드문 원인이지만증례로 보고된 경우는 임신과 연관된 자간전증 또는 전자간증 및 hemolysis, elevated liver enzymes, low platelets (HELLP) 증후군 등이 있으며 간세포암종, 간샘종 및 간의 악성 및 양성 종양의 파열과의 연관성이 보고된 바가 있다. 또는 의학적시술, 감염, 약물에 의한 간자색반병도 일부 보고되어 있다.
본 증례는 외상이나 응고 장애 등이 동반되지 않는 상태에서 반복적으로 피막하 혈종이 발생하였으며, 여러 문헌고찰을 통해 확인된 가능한 원인들에 대해 감별하였으나 뚜렷한원인을 밝혀낼 수는 없었다. 다만, 가능성을 완전히 배제하기어려웠던 기생충 감염에 대해 항구충제 치료 및 타목시펜 복용과 연관된 간자색반병의 가능성에 대해 타목시펜 중단 등의 조치를 취하였으며, 이후 반복적으로 발생하던 경과가 임상적으로 호전되는 양상을 보였다. 이에 비록 관찰 기간이충분하게 길지 않지만, 문헌고찰과 함께 보고하는 바이다.
