Purpose
The Oncotype DX 21-gene recurrence score (21-gene RS), which is most widely used multi-gene test to predict a prognosis and determine adjuvant chemotherapy, is known to be associated with endocrine resistance in ER+/HER2- breast cancer. Several lines of evidence support that TP53 mutation is associated with endocrine resistance in ER+ breast cancer. To date, there is no report regarding the relationship between TP53 mutation and 21-gene recurrence score. We investigated whether TP53 mutation is linked with 21-gene RS in patients with ER+/HER2- breast cancer. We further evaluated molecular characteristics of ER+/HER2- breast cancer according to TP53 mutation using the TCGA.

Method
We identified 141 patients who underwent TP53 gene sequencing and Oncotype DX test between August 2011 and March 2020 at a single institute. Mutational analysis of the TP53 gene was carried out using TP53 Sanger direct sequencing or targeted NGS with the Oncomine™ comprehensive panel (Thermo Fisher Scientific, Waltham, MA, USA). Clinicopathological variables were compared. Multivariable binary logistic regression analysis was performed to identify predictive factors for high 21-gene RS (≥26).
