Clinical trials are research studies that explore whether a strategy, treatment, or device is safe and effective for humans. Randomized clinical trial (RCT) has been believed as the only valid method for comparing treatment and crowns the pyramid of evidence. However, poorly conducted RCTs are likely to result in unwarranted credibility. Surgery seems to be such an area. Surgery has traditionally been more experience based than evidence based, and much achievements of surgical treatment have taken place without scientific trials. Most drugs have been applied in practice after smaller phase II and larger phase III trials are carried out. Conversely, surgical RCTs are usually not done in stages so appropriate endpoints, effective size and statistical power can therefore be difficult.
