Objectives: The aim of the study was to develop the osteoporosis risk scorecard model in korean adult women aged over 30 years old. Methods: The participants of the study were 11,427 adult women aged over 30 years old who participated in the four years of the Korea National Health and Nutrition Examination Survey (2015-2018). The data were analyzed using Rao-Scott chi-square test and weighted multiple logistic regression analysis of complex sampling design. The osteoporosis risk scorecard model was developed by weighted multiple logistic regression using points to double the odds (PDO) method. Results: This study found that there were many major influencing factors for osteoporosis which included household income, educational level, body mass index (BMI), hypertension, stroke, rheumatoid arthritis, hypercholesterolemia, sarcopenia, menopause and age of menarche. In the scorecard results, the highest score range was observed in the order menopause, age of menarche (16-17 years old), educational level (elementary school or below), rheumatoid arthritis BMI ( < 18.5 kg/m2 ) and others, suggesting that these were the most important risk factors. Conclusions: The significance of this study is that it comprehensively grasps the causal relationship between osteoporosis and various factors in korean adult women. Also, it can be used as a useful measurement tool for women’s musculoskeletal health management in health-related organizations by scoring this relationship
