As about 90% of all diabetic patients is consisted of Non-Insulin Dependent Diabetes Mellitus (NIDDM, type II diabetes), it is important to study how insulin's ability is decreased and how insulin action can be improved in these patient as well as in healthy people. Insulin resistance, one of the most prominent metabolic symptoms in type II diabetes, is a condition in which insulin action is lower than normal, an increase in insulin action is the most critical factor to maintain normal blood glucose level in type II diabetic population. One way to improve insulin action is exercise (or muscle contraction). There is an evidence that 5'AMP-Activated Protein Kinase (AMPK) and glycogen concentration might be associated with skeletal muscle insulin sensitivity. These two factors are influenced by exercise or muscle contraction such that activity of AMPK is increased, and glycogen is utilized and its concentration is decreased by muscular activity. Thus, an increase in AMPK activity and decrease in glycogen concentration may affect insulin sensitivity in skeletal muscle. However, the exact cellular mechanism by which these two factors are associated with muscle insulin sensitivity is not known. Furthermore, how insulin signaling pathway post exercise can be altered to increase insulin action is still not known.
