포도막흑색종은 성인에서 가장 흔한 원발성 안내종양이다. 국내에서는 연간 백만 명당 0.42명이 발생하는 것으로 보고되었으며, 점차 발생률이 증가하고 있다. 포도막흑색종의 90%는 맥락막에서 발생하며 대부분 둥근 모양(dome?shaped)으로 관찰된다. 원발 종양 치료의 근간은 방사선이며, 특히 중간 크기 종양의 표준치료는 근접방사선치료이다. 원발 종양의 경우 치료에 잘 반응하나 장기적으로 약50% 환자에서 전신 전이가 관찰되며 간 전이가 가장 흔하다. 아직까지 포도막흑색종의 발생 및 전이에 대한 이해가 부족하며 전이성 포도막흑색종에 대한 효과적인 치료방법은 개발되지 않았다. 최근 연구를 통해 (GNAQ, GNA11, PLCB4, CYSTLR2) 유전자의 유발변이(driver mutation)와 이로 인해 활성화되는 발암신호전달경로(oncogenic signaling pathway)에 대한 여러 연구가 활발히 이루어지고 있으며 이를 기반으로 표적치료(targeted therapy)가 시도되고 있다. 본 종설에서는 한국인 포도막흑색종에 대한 임상 경험을 공유하고, 분자유전학적 특성 및 치료에 대한 최신 연구 동향을 포괄적으로 고찰하고자 한다.
