Purpose : To analyze changes in high-order aberrations (HOA) after wearing aspheric multifocal contact lenses (MFCL) for myopic eyes. Methods : Thirty-three healthy adults (mean age: 3.03±2.89 years, mean equivalent spheric power: -3.28±1.67 D, 66 eyes) with no history of ocular surgery, and systemic disease were enrolled. After wearing single vision contact lens (SVCL) and low and high add MFCL (Biotrue, Bausch + Lomb, USA) HOAs, coma, and spherical aberrations (SA) were measured in a scotopic (0 lx) and photopic (20 lx) condition using Wavefront Analyzer (KR-1W, Topcon, Japan). Statistical analysis was performed using SPSS (version 21.0). Results : Total HOAs increased in the eyes wearing MFCL (Low and High addition) compared to the naked and SVCL wearing eyes (p=0.012, p<0.001), and it increased more with high add MFCL. Coma aberration increased in the eyes wearing SVCL and MFCL compared to the naked eye (p<0.001, p<0.001). The SA in the eyes wearing MFCL changed to the negative direction and much more with the high add MFCL. Conclusion : After wearing aspherical MFCL, the total HOA and coma aberration increased, and the SA changed to the negative direction. According to individual aberrations of the presbyopia wearing aspheric MFCL, the negative SA of MFCL could be compensated or enforced that MFCL addition effect can be decreased and visual quality may be decreased due to coma aberration.
