The introduction of innovative medical technology requires changes in health technology assessment and coverage decision-making. The pace of medical technology development is accelerating, and existing entry criteria of coverage decision making need to change. Interest in coverage with evidence development (CED) policy is increasing in major countries. Since the 1990s, various CEDs have been operated by country (USA, UK, Australia, Japan, etc.). The objectives of this study reviews CED in the coverage decision-making process and examines current status of CED operations in Korea. In addition, we would like to review the issues that should be considered in operating the efficient CED in the future. This is a system that provides temporary benefits for new medical technologies when the evidence for the technologies is uncertain, and provides benefits for a certain period of time until the evidence is established. Also, it is a system that produces additional evidence to reduce uncertainty about clinical effectiveness and cost-effectiveness, and secures the potential effectiveness of technology and patient accessibility. The implementation process of CED in major countries has various conditions for each technology item. In generating evidence, the importance of the process of collecting research data, setting indicators, and deriving results is emphasized. It is important to establish guidelines for this process and specify the implementation process. Therefore, the conditional payment system requires a clear process for conducting research and monitoring. Currently, Korea is also operating a conditional selective benefit system, and it is necessary to prepare specific plans for the efficient implementation system and decision-making system related to conducting research to generate evidence, system operation, and evaluation processes.
