Objectives: This study aims to investigate effectiveness of herpes zoster vaccines. Methods: Systematic searches on OVID-Medline and KoreaMed were performed on March 6th 2023. Randomized controlled trials (RCTs) and retrospective studies (RSs) were included, and a qualitative analysis was conducted. The outcomes of interest included protection against herpes zoster and related complications. Results: A total of six RCTs and twelve RSs were included in the analysis: one RCT and eight RSs of zoster vaccine live (ZVL), and five RCT and four RSs of recombinant zoster vaccine (RZV). Protection against herpes zoster by ZVL was approximately 50% in one year after vaccination, and the effectiveness decreased to 14%–39% after five years. Protection of herpes zoster by RZV was maximum 89% in two years, and the effectiveness decreased to 84% after 7–8 years and 73% after ten years. Conclusion: Compared to ZVL, RZV was judged to be more effective in protection against herpes zoster and showed a smaller decrease over time. We expect that this information can serve as a basis for policy making processes regarding the national immunization program in Korea.
