This study is a descriptive research study to understand the effect of health literacy and stroke self-efficacy of first stroke patients on the health behavior adherence. The participants included 96 first stroke patients who visited the outpatient clinic of a higher general hospital located in W city. Data were analyzed with descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and multiple regression, using the SPSS program 28.0. As a result of the study, the health behavior adherence in first stroke patients had a significant positive correlation with stroke self-efficacy (r=.72, p <.001). The regression model was statistically significant (F=56.46, p <.001), and the explanatory power for health behavior adherence was 53.9%. Stroke self-efficacy (β=.69, p <.001), and age (β=.17, p =.023) significantly influenced health behavior adherence among first stroke patients. Based on the above results, to improve health behavior adherence in first stroke patients, various strategies are needed to strengthen their stroke self-efficacy while considering their age.
