Objective : To evaluate that the anodal tDCS over the lesioned leg motor cortex can enhance strength and coordination of contralateral lower extremity and this effect could enhance the postural stability of hemiplegic subject.
Material & Methods : Eleven ambulatory hemiplegic patients were included in the study. Anodal or sham stimulation on lesioned cortex of lower extremity was delivered. The stimulation intensity was 2mA. The experimental period was three days. All subjects took part in 10 min of two tDCS sessions consisting of anodal stimulation and sham stimulation. The order was counter-balanced among subjects. Before and after each stimulation session, static postural stability was evaluated in the condition of subject''s eye opened and closed. Also, the isometric strength of hemiplegic side of knee was measured before and after each stimulation session. And repeated measure ANOVA were used to determine the statistical significance of improvement in postural stability and strength.
Result : There was significant improvement for overall stability index with eye opened and closed after anodal tDCS(p<0.05). And the isometric strength of lesioned quadriceps tended to increase after anodal tDCS(p<0.05). The postural stability and quadriceps strength were not changed after sham stimulation.
Conclusion : The anodal tDCS might be used in the hemiplegic stroke patient for the purpose improvment of balance. In the aspect of strengthening the lower extremity, it is demanding further clinical study with more subjects. And it is thought that there would be another factors affecting the postural stability.
