본 연구는 치열교정환자의 전문가 칫솔질에 따른 치면세균막 관리 효과를 평가하여 주기적이고 체계적인 전문가 칫솔질 구강보건교육 프로그램의 활성화에 기여하고자 하였다. 본 연구는 IRB 승인 후 2017년 5월 15일부터 9월 30일까지 부산광역시 소재의 교정전문 E 치과 내원 환자 중 자발적으로 연구 참여에 동의한 환자 최종 30명을 대상으로 치면세균막 관리 효과를 비교하기 위하여 실험군 15명과 대조군 15명으로 분류하여 설문 조사 및 구강검사를 실시하였다. 실험군에서 1주 간격으로 총 4회 전문가 칫솔질 및 구강보건교육을 시행하였고, 대조군에서는 동일한 기간과 횟수로 구강보건교육을 시행한 결과 다음과 같은 결론을 얻었다. 전문가 칫솔질 및 구강보건교육에 따른 구강보건지식, 태도, 행동 차이를 분석한 결과, 실험군과 대조군에서 구강보건교육 전과 후 모두 높게 나타났으며 구강보건교육을 시행한 대조군보다 전문가 칫솔질과 구강보건교육을 병행한 실험군에서 구강보건지식, 태도, 행동이 모두 높게 나타났다(p<0.05). 구강보건지식, 태도, 행동의 상관관계를 분석한 결과 구강보건태도와 행동 간(r=0.971, p<0.01)에는 유의한 양의 상관관계가 있는 것으로 나타났고, 구강보건태도가 높을수록 행동도 높게 나타났다. 전문가 칫솔질 시행시기에 따른 치면세균막지수 변화를 분석한 결과, 시행시기에 따른 치면세균막지수(F=21.895, p<0.001)와 집단 간 치면세균막지수(F=4.477, p=0.043)에 유의한 차이가 있었으며, 실험군은 대조군보다 전문가 칫솔질 시행시기에 따른 치면세균막지수가 유의하게 낮은 것으로 나타났다. 전문가 칫솔질 시행시기에 따른 치은출혈지수 변화를 분석한 결과, 시행시기에 따른 치은출혈지수에 유의한 차이가 있었으나(F=3.424, p=0.021), 집단 간 치은출혈지수는 차이가 없었다. 측정 시점별 치면세균막지수 및 치은출혈지수 차이를 분석한 결과, 실험군 및 대조군 간에는 3주, 4주에서 통계적으로 유의하였다(p<0.05). 본 연구에서 전문가 칫솔질과 구강보건교육을 치열교정환자에게 시행 한 결과 치면세균막 관리 효과가 확인되었다. 따라서 치열교정환자의 구강건강증진을 위한 전문가 칫솔질 및 구강보건교육 프로그램이 적극 적용되고 확산될 수 있는 방안이 모색되어져야 할 것이다.
The purpose of this study was to contribute to develop an oral health education program for organized professional toothbrushing by determining the effect of professional toothbrushing on plaque control in patients undergoing orthodontic treatment. The protocol of this study obtained approval from Institutional Review Board (IRB). The subjects were 30 patients from outpatients of E orthodontic dental clinic located in Busan Metropolitan City who agreed voluntarily to study participation and were assigned to experimental group (n=15) and control group (n=15) and the effects on plaque removal were compared using questionnaire survey and oral examination from May 15th to Sep. 30th, 2017. The subjects of experimental group participated in professional toothbrushing and oral health education weekly for three weeks and those of control group participated in same program without professional toothbrushing education. The results of this study were as follows: The analysis of difference in oral health knowledge, attitude, behavior scores between pre- and post-intervention showed that the differences were high in both experimental and control groups and that the post-intervention scores were higher in experimental group compared to control group for all sub-factors, oral health knowledge, attitude, and behavior. The analysis of association among oral health knowledge, attitude, and behavior showed that there is significant positive correlation between oral health attitude and behavior (r=0.971, p<0.01), indicating that those with higher oral health attitude score were likely to have higher oral health behavior score. The analysis of the effect of professional toothbrushing time on plaque index showed that there were significant difference in plaque index between toothbrushing time (F=21.895, p<0.001) and groups (F=4.477, p=0.043) and that the effect of toothbrushing time on the plaque index was significantly lower in experimental group compared to control group. The analysis of the effect of professional toothbrushing time on sulcus bleeding index showed that there were significant difference by time (F=3.424, p=0.021), but not by groups in sulcus bleeding index. The analysis of the effect of measurement time on plaque index and sulcus bleeding index showed that the difference between experimental and control groups were two and three weeks intervention (p<0.05). The results of this study demonstrated the positive effect of professional toothbrushing and oral health educations on plaque control in patients undergoing orthodontic treatment, suggesting that it is necessary to seek a measure to implement the education program for oral health and professional toothbrushing to improve oral health.
