고유수용성신경근촉진법에 의한 복부강화 훈련이 만성 허리통증 요인들에 미치는 영향

본 연구는 복부강화 훈련이 허리통증, 골반기울기, 몸통근육두께 및 기능적 활동에 미치는 영향을 알아보기 위해 12주 이상의 만성 허리통증을 가진 35∼45세 연령범위의 환자 30명을 대상으로 하였다. 이들을 각각 고유수용성신경근촉진법에 의한 복부강화 훈련을 실시한 집단 10명과 일반적인 복부강화 훈련을 실시한 집단 10명 및 일반 물리치료를 실시한 집단 10명으로 무작위배정 하였다. 실험군Ⅰ과 실험군Ⅱ 및 대조군은 주 3회, 6주간 중재를 실시하였다. 각 집단은 훈련 전과 6주 후에 평가를 실시하여 집단 내·간 훈련 효과를 알아보았다.
통증정도는 시각적 통증 사상척도(visual analogue scale, VAS)를 이용하여 측정하였고, 기능적 활동의 변화는 한국어판 버그균형척도(Berg`s balance scale, BBS)와 한국어판 오스웨스트리 허리통증 장애지수(Korean Oswestry Disability Index, ODI)로 측정하였다. 골반기울기는 Dartfish 소프트웨어를 이용하여 측정하였고, 몸통근육두께는 초음파영상장비(LOGIQ S7, GE Healthcare, Little Chalfont, UK)를 이용하여 배바깥빗근, 배안쪽빗근, 배가로근, 뭇갈래근을 측정하였다.
중재 전·후 각 집단 내 차이를 비교하기 위하여 대응표본 t-검정(paired t-test)을 사용하였고, 중재 후 집단 간의 훈련에 따른 차이를 비교하기 위하여 일원배치 분산분석(one-way ANOVA)을 사용하였다. 사후검정으로 Bonferroni를 사용하여 다음과 같은 결과를 얻었다.
허리통증의 변화에서 실험군Ⅰ과 실험군Ⅱ 및 대조군은 집단 내 통증의 변화에 유의한 차이가 있었고(p<.05), 집단 간 통증 변화에서 유의한 차이가 있었으며(p<.05), 실험군Ⅰ과 실험군Ⅱ는 대조군보다 통증의 감소가 더욱 효과적으로 나타났다. 골반기울기 변화는 실험군Ⅰ과 실험군Ⅱ 및 대조군은 집단 내 골반기울기의 변화에 유의한 차이가 있었고(p<.05), 집단 간 골반기울기 변화에서 유의한 차이가 있었으며(p<.05), 실험군Ⅰ이 대조군 보다 골반기울기 변화가 더욱 효과적으로 나타났다. 근육 두께 변화는 배바깥빗근의 변화는 실험군Ⅰ과 실험군Ⅱ 및 대조군에서 집단 내 유의한 차이가 있었고(p<.05), 집단 간 배바깥빗근의 변화는 유의한 차이가 없었다. 배안쪽빗근의 변화는 실험군Ⅰ과 실험군Ⅱ 및 대조군에서 집단 내·간 유의한 차이가 없었다. 배가로근의 변화는 실험군Ⅰ과 실험군Ⅱ 및 대조군에서 집단 내 유의한 차이가 있었고(p<.05), 집단 간 배가로근의 변화에서 유의한 차이가 있었으며(p<.05), 실험군Ⅰ과 실험군Ⅱ가 대조군보다 배가로근 변화가 더욱 효과적으로 나타났다. 뭇갈래근의 변화는 실험군Ⅰ, 실험군Ⅱ, 대조군에서 집단 내 유의한 차이가 있었고(p<.05), 집단 간 뭇갈래근의 변화에서 유의한 차이가 있었으며(p<.05), 실험군Ⅰ과 실험군Ⅱ가 대조군보다 뭇갈래근의 변화가 더욱 효과적으로 나타났다. 균형능력은 실험군Ⅰ과 실험군Ⅱ 및 대조군은 집단 내 균형능력의 변화에 유의한 차이가 있었고(p<.05), 집단 간 균형능력 변화에서 유의한 차이가 있었으며(p<.05), 실험군Ⅰ과 실험군Ⅱ이 대조군보다 균형능력 변화가 더욱 효과적으로 나타났다. 허리통증 장애지수는 실험군Ⅰ, 실험군Ⅱ, 대조군은 집단 내 허리통증 장애지수의 변화에 유의한 차이가 있었고(p<.05), 집단 간 허리통증 장애지수의 변화에서 유의한 차이가 있었으며(p<.05), 실험군Ⅰ이 대조군보다 허리통증 장애지수 변화가 더욱 효과적으로 나타났다. 실험군 Ⅰ의 6주 동안 종속변인들 간의 상관관계 결과, 균형능력이 증가할수록 배바깥빗근(r=-.734), 배안쪽빗근(r=-.674)은 음의 상관관계를 보였다(p<.05). 배바깥빗근의 두께가 증가할수록 배안쪽빗근(r=.942), 뭇갈래근(r=.694)은 양의 상관관계를 보였다(p<.05). 배안쪽빗근의 두께가 증가할수록 뭇갈래근(r=.749)은 양의 상관관계를 보였다(p<.05). 실험군Ⅱ의 6주 동안 종속변인들 간의 상관관계 결과, 골반기울기가 감소할수록 배안쪽빗근(r=.754), 뭇갈래근(r=.813)은 양의 상관관계를 보였다(p<.05). 배바깥빗근의 두께가 증가할수록 배안쪽빗근(r=.901), 뭇갈래근(r=.920)은 양의 상관관계를 보였다(p<.05). 배안쪽빗근의 두께가 증가할수록 뭇갈래근(r=.929)은 양의 상관관계를 보였다(p<.05). 대조군의 6주 동안 종속변인들 간의 상관관계 결과, 골반 기울기가 감소할수록 배안쪽빗근(r=.817)은 양의 상관관계를 보였다(p<.01). 배안쪽빗근의 두께가 증가할수록 배가로근(r=.820), 뭇갈래근(r=.826)은 양의 상관관계를 보였다(p<.05). 배가로근의 두께가 증가할수록 뭇갈래근(r=.855)은 양의 상관관계를 보였다(p<.05).
본 연구 결과를 바탕으로 만성 허리통증환자에게 고유수용성신경근촉진법을 이용한 복부강화 훈련이 통증감소, 골반기울기 감소, 균형능력 향상 및 허리통증 장애지수 감소가 일반적인 복부강화 훈련보다 더 효과적으로 나타났다. 따라서 만성 허리통증환자에게 고유수용성신경근촉진법을 이용한 복부강화 훈련은 임상에서 보다 효율적인 중재방법으로 사용될 수 있을 것으로 생각된다.
