본 연구는 종합병원 입원환자의 낙상 위험 수준별 낙상 발생 예측요인을 규명하고자 시도되었다.
연구 자료는 2017년 1월부터 2019년 12월까지 B시에 소재하는 D 종합병원에 입원한 18세 이상 환자 중 낙상군 170명, 비낙상군 340명으로 총 510명의 전자의무기록을 통해 수집하였다.
수집한 자료는 SPSS WIN 24.0 프로그램을 이용하여 분석하였다. 낙상 위험 수준에 따른 일반적 특성은 χ²-test, Fisher’s exact test로 분석하였다.
낙상 위험 수준별 낙상 발생 예측요인을 분석하기 위해 로지스틱 회귀분석을 실시하였다.
연구의 결과는 다음과 같다.
1. 전체 입원환자의 낙상군과 비낙상군은 지남력장애(χ2=23.13, p=.001), 전신쇠약(χ2=3.85, p=.050), 마비(χ2=6.89, p=.009), 보행장애(χ2=10.87,p=.001), 변완화제(χ2=4.23, p=.040), 항정신성약물(χ2=4.49, p=.034), 혈당강하제(χ2=7.19, p=.007), 진정제(χ2=10.00, p=.002), 보호자 동반 여부(χ2=4.35, p=.037)에서 유의한 차이가 있었다. 낙상 발생 예측요인으로 지남력장애(OR=3.48, 95%CI=1.75-6.90, p<.001), 전신쇠약(OR=2.63, 95%CI=1.04-6.61, p=.040), 보행장애(OR=1.67,95%CI=1.01-2.78, p=.047), 혈당강하제(OR=1.98, 95%CI=1.25-3.13,p=.004), 진정제(OR=15.80, 95%CI=3.06-81.56, p=.001), 보호자 동반(OR=0.43, 95%CI=0.28-0.66, p<.001)이 확인되었다.
2. 낙상저위험군의 낙상군과 비낙상군은 수면장애(χ2=4.91, p=.048), 배설장애(p=.037), 변완화제(χ2=10.52, p=.005)에서 유의한 차이가 있었다.
낙상 발생 예측요인으로 배설장애(OR=12.49, 95%CI=1.16-134.34,p=.037)와 변완화제 복용(OR=9.62, 95%CI=2.14-43.22, p=.003)이 확인되었다.
3. 낙상중위험군의 낙상군과 비낙상군은 지남력장애(χ2=9.66, p=.002), 마비(χ2=7.30, p=.007), 보행장애(χ
2=5.92, p=.015), 항정신성약물(χ2=6.83, p=.009)에서 유의한 차이가 있었다. 낙상 발생 예측요인으로 지남력장애(OR=2.40, 95%CI=1.11-5.19, p=.026)가 확인되었다.
4. 낙상고위험군의 낙상군과 비낙상군은 지남력장애(χ2=8.33, p=.008), 혈당강하제(χ2=9.14, p=.003)에서 유의한 차이가 있었다. 낙상 발생 예측 요인으로 지남력장애(OR=12.54, 95%CI=1.39-113.26, p=.024), 혈당강하제 복용(OR=5.39, 95%CI=1.58-18.44, p=.007)이 확인되었다.
본 연구결과를 통하여 낙상 위험 수준별 낙상 발생 예측요인이 다르게 나타났으며, 중위험군과 저위험군에서도 낙상 예방활동이 필요함을 확인하였다. 그러므로 낙상 위험 수준별 낙상 위험 요인을 파악하여 낙상 예방 및 교육에 관한 간호중재 방안을 모색하고, 좀 더 정확히 예측할 수 있는 낙상 위험평가 도구 개발을 위한 기초자료로 활용될 수 있을 것이다.
