Objectives : The purpose of this study is to investigate the effect of leptin on alcohol craving in patient with alcohol dependence. We investigated the relationship between plasma leptin level and craving degree of patients with alcohol dependence according to abstinence duration. We also investigated effects of alcohol on hypothalamic-pituitary-adrenal(HPA) axis and relationships between plasma leptin levels and HPA axis.
Methods : Nineteen patients who met DSM-Ⅳ criteria for alcohol dependence and 10 normal controls were enrolled in this study. In all subjects, the levels of plasma leptin, cortisol, and ACTH were measured at 8 AM in the first day and the 14th day of abstinence. Obsessive Compulsive Drinking Scale(OCDS) was also tested in same days to estimate the degree of their craving.
The SPSS version 10.0 for window was used for statistical analysis.
Result :
1) In comparison with normal control group, patient group showed significantly low plasma level of leptin. There was also a significant decrease of plasma leptin level in the 14th day of abstinence compared with the level in the 1st day. Moreover, significant correlation was found between plasma leptin level and OCDS score.
2) Patients with alcohol dependence showed significantly higher plasma levels of cortisol and ACTH than control group in the first day of abstinence. However, there was no significant difference in the 14th day.
3) There was a significant correlation between plasma leptin level and plasma cortisol level in patients with alcohol dependence. There was a tendency of decrease in plasma ACTH level when plasma leptin was decreased, but there was no statistically significant correlation.
Conclusion : The study showed that alcohol stimulated the secretion of HPA axis hormone and that plasma leptin level was significantly correlated with cortisol level. Moreover, this study showed also that plasma lectin levels of patients with alcohol dependence was lower than that of control group and was significantly correlated with score of obsessive compulsive drinking scale. We suggested that leptin might be a biological etiology of alcohol dependence and be a factor of their relapse through the regulation of craving.
