본 연구는 요추 디스크절개술 환자를 대상으로 복합운동과 추가적인 고유수용감각훈련을 시행했을 때 요부의 자세조절 능력과 통증에 미치는 영향을 알아보는데 목적이 있다. 피험자 모집은 요추 디스크 절개술 4주 후 요부 통증만 남아있는 환자 38명을 대상으로 각각 18명씩 요부 복합운동그룹(N-LPROG), 요부 복합운동과 고유수용감각을 추가한 운동그룹(LPROG)으로 나누어 10주간 주 3회로 운동 프로그램을 실시하였고 두 그룹 간 굴곡과 신전의 ROM, 자세조절 능력, 요부신전근력, 요 부 통증(VAS)을 측정하여 비교하였다. 그 결과 두 그룹 모두 운 동전에 비하여 운동 후에 굴곡과 신전의 ROM, 자세조절 능력, 요부신전근력, 요부 통증(VAS) 모두 유의하게 증가하였으나, 두 그룹 간에는 LPROG에서 N-LPROG 보다 더 효과적인 것으로 나타났다(p<. 05). 따라서 성공적인 요추 디스크 절개술 후에도 요부 재활운동이 필요하며, 특히 심 부와 표층근육 (deep and superficical muscle) 의 조절훈련 외에 자세조절 능력을 증가시키는 다양한 고유수용감각 훈련을 필요하다고 판단 된다.
The purpose of this study is to identify what effects lumbar extensor strength, lumbar ROM and pain scale, when applying lumbar exercise program. This is used for chronic low back pain obese patients, which is one of the various people with back pain. The subjects consist of 45, diagnosed with chronic back pain, randomly classifying experiment group(13 males, 9 females) and control group(13 males, 10 females); all group of participants who had lumbar exercise programs for 3 times a week, 50~55 minutes, total 8 weeks, respectively. Lumbar extensor strength, lumbar ROM and pain scale is tested with Medx lumbar extension machine, ROM plus and VAS. Results are as follows: 1. Lumbar extensor strength in experiment (AO: Abdominal obesity) and control (NAO: None-abdominal obesity) group shows significant differences in every angle (p<.001). 2. Compared in between experiment group and control group, an average gap of Lumbar extensor strength, all extensor strength angle shows significant differences more experiment group than control group.(when 0˚ p<.05, 12˚ p<.01, 24˚ p<.05, 36˚p<.05, 48˚ p<.01, 60˚ p<.05, 72˚ p<.01) 3. Lumbar ROM (flexion & extension) in experiment (AO) and control group shows significant differences in every angle (p<.001). 4. Compared in between experiment group and control group, an average gap of Lumbar ROM, lumbar ROM shows significant differences more experiment group than control group.(when flexion angle p<.001, extension angle p<.05) 5. Pain scale (VAS) in experiment (AO) and control (NAO) group shows significant differences in every angle (p<.001). 6. Compared in between experiment group and control group, an average gap of Lumbar ROM, pain scale shows significant differences more experiment group than control group(p<.0.032). This study shows lumbar exercise programs have effect on improving lumbar extensor, lumbar ROM and decreasing pains to AO and NAO patients. Especially, it shows that NAO improved better than AO. However, as only obesity and none obesity chronic back pain patients are subjected to it, there will be a little to go far that this study is applied to all back pain patients. Furthermore, one can suggest that a more various research, developing systemic exercise program tailored to individual patients, should be done.
