This article explored theoretical and empirical basis supporting the values of motor imagery and action observation as cognitive intervention strategies for facilitating the rehabilitation of stroke patients. Therapeutic approaches for the recovery of motor functions of stroke patients have the characteristics of being applied to the affected side with the purpose of improving its functioning. However, those approaches face considerable difficulties in clinical situations, since stroke patients have limited capacity to use the affected limb. In order to increase the efficiency of rehabilitation programs for stroke patients, a cognitive intervention is required that can induce active participation of patients in the rehabilitative activities while facilitating the effect of physical therapy. Motor imagery used as cognitive intervention for enhancing the effect of rehabilitative programs has been found to positively affect the functional recovery of stroke patents. However, the fact that a damage to certain brain areas may decrease the ability of imagery as well as physical functioning raises doubts for applying motor imagery to the rehabilitation of stroke patients. Cognitive intervention for stroke patients has to provide clear and specific stimulus on the activities of affected limb regardless of patients' imagery ability, and previous studies on the activation patterns of brain areas during observational learning suggest that action observation can be a candidate for such a role. Action observation can contribute to the reorganization of damaged brain areas by activating cognitive mechanisms used in physical activities, and, thus, may facilitate motor performance and learning of stroke patients.
