The purpose of this study was to assess the fatigue in lumbar and abdominalmusc1es in patients with chronic low back pain compared with normal subjects usingspectral analysis with mean power frequency and median power frequency. Theexperimental group consisted of twenty subjects who had experienced chronic low backpain for over one year after the onset day. A control group consisted of twenty normalsubjects with no history of low back pain. All subjects stood in an apparatus toperform sustained contraction in the lumbar and abdominal muscles for 30 seconds with60% maximal voluntary isometric contractíon (MVIC). The resulting electromyographic(EMG) recorded time serial data were transformed into frequency serial data by FastFourier Transformation (FFT). The results were as follows: 1) lumbar musclesmeasured, the frequency change ratio of both median power frequency and mean powerfrequency was significantly greater for experimental group compared with control groupgroup (p<0.05). In measured two abdominal muscles Cinferior rectus abdominis. obliquus extemus abdomi띠s) except superior rectus abdominis, the frequency change ratio of both median power frequency and mean power frequency was significant1y greater forexperimental group compared with control group (p<0.05). 2) In all three Oongissimusthoracis, iliocostalis lumborum, multifidus) lumbar muscles measured, the initialfrequency value of both median power frequency and mean power frequency wassignificant1y lower for the experimental group compared with the control group (p <0.05). In the two Onferior rectus abdominis, obliquus extemus abdominis) abdominalmuscles measured (superior rectus abdominis not included), the initial frequency valueof both median power frequency and mean power frequency was significant1y lower forthe experimental group compared with the control group (p<0.05). These resultssuggest that in patients with chronic low back pain there is a trend for more fatigue tooccur in both lumbar and abdominal muscles than in the normal control group. Thiswould seem to suggest that in treatment programs for patients with chronic low backpain, improvement of endurance in all trunk muscles should be considered.
