The recommendations for systemic treatment of specific molecular subtypes of breast cancer are distinct. However, surgery remains the cornerstone of treatment. Although the use of chemotherapy for luminal type breast cancers has reduced in recent years, use of neoadjuvant chemotherapy (NAC) has been increasing for patients who need chemotherapy eventually. This is particularly observed among those with triple-negative or HER2-positive breast cancer, in which pathologic complete response (pCR) rates are observed to be as high as 50–70%. As suggested by the results from the CREATE-X and KATHERINE trials, in addition to the benefits of downstaging for less extensive surgery and assessing chemo-sensitivity in vivo, NAC allows the assessment of residual disease. This may be used as a biomarker to tailor treatment based on pCR.

At present, surgery of the breast and axilla remains the standard of treatment following NAC, irrespective of clinical response. A number of studies conducted in the 1970–90 s evaluated definitive local radiotherapy without surgery in patients with a complete clinical response (cCR) after NAC. However, all studies reported unacceptable locoregional failure rates, indicating that surgical resection was essential despite a good response to NAC.
