Prolonged orotracheal intubation of preterm infants in pediatric intensive care units can lead to various complications in oral cavities, including morphological deformations of the palate and maxillary arch as well as developmental dental defects. This report describes 9-month-old boy with a deformity due to 9 months of orotracheal intubation. The patient was a preterm infant born at 28 weeks’ gestation with an extremely low birth weight. He received assisted mechanical ventilation because of bronchopulmonary dysplasia. Concave alveolar ridge notching and palatal grooving were observed along the fixed position of the tube. Additionally, constriction of the palatal width was detected. A simple splint was fabricated to protect the deformed alveolar ridge and palate and restore its original shape. The appliance was used for 2 months before and after extubation and the deformed area returned to its normal contour. In preterm infants with long intubation periods, a good prognosis without complications may be expected with the use of this simple appliance.
