Japan and Korea are the most nearest countries geographically and have similar cultures, but different socioeconomically. The purpose of this study was to compare activity fitness of daily living (AFDL) and its components over a wide age range between Korean and Japanese elderly women. Four hundred and nineteen older women (228 Koreans and 191 Japanese), aged from 65 to 86 years served as subjects. Twelve performance test items related to the AFDL were measured in Japan and Korea by the same investigators, using the same instruments. The Pearson's correlation coefficients between the performance test items and age were significant and the score of all items remarkably decreased with aging. Koreans had significantly lower functional fitness scores than Japanese in most items, although the magnitude of this difference depended on the items. Insignificant differences (P>0.05) were found in flexibility and upper extremity muscle endurance factors between Korean and Japanese. To identify potential dimensions of overall activity fitness, principal component analysis was applied to a 12 × 12 correlation matrix. The results of the comparison clearly indicated that the Korean elderly women were inferior to Japanese elderly women in the total AFDL score. The results of the comparison with the AFDL components between Japanese and Korean elderly women showed that the Japanese elderly women were significantly superior in agility of the hand and foot in a seated position, strength and movement of the body, and balance. From these results, it can be concluded (1) that advancing age significantly affects performance for all items in older women of both countries, (2) that the higher AFDL level would contribute to a lifestyle of more engagement in exercise or physical activity with a longer duration and a higher frequency in Japanese older women.
