전립선암의 전이는 일반적으로 혈청 PSA의 현저한 상승을 동반하지만 PSA값의 상승을 보이지 않을 수도 있다. 따라서 기존에 근치적 전립선 수술을 받은 환자에서 PSA상승을 동반하지 않으면서 전이암 소견을 보이는 경우나 원발 불명암 환자에서 PSA 상승이 없는 경우에 전립선암 감별을 위해 관련 면역조직화학염색이나 다른 혈청 표지자 등을 이용한 추가 감별 시도가 필요하다.
