전방십자인대 재건술에 있어서 최근 가장 각광받고 있는 개념은 해부학적 전방십자인대 재건술이다. 경경골 술식을 이용한 고전적인 방법에 비하여 해부학적 재건술을 위해서 경골터널은 이전보다 전방으로 이동하며 대퇴터널은 낮고 비스듬한 방향으로 만들게 된다. 따라서 낮고 비스듬한 해부학적인 위치의 대퇴터널을 만들기 위하여 전내측 도달법을 통한 술식(anteromedial portal technique), outside-in 술식, 그리고 변형된 경경골 술식(modified transtibial technique) 등이 시행되고 있다. 각각의 방법은 장단점이 있어 술자의 선호도, 경험, 사용 가능한 기구 및 이식건의 선택 등에 따라 선택된다.
