RANKL의 인간 단일클론 항체인 denosumab은 골다공증 치료제로 사용되고 있다. Denosumab은 파골세포의 분화와 작용을 억제하여 골밀도를 증가시키고 척추, 고관절, 비척추 골절에 대한 예방효과를 나타낸다. FREEDOM 연구와 이를 10년까지 연장한 FREEDOM Extension 연구 등의 임상연구를 통하여 폐경 후 골다공증에 대한 denosumab 장기투여의 효과와 안정성이 확인되었다. 또한 남성 골다공증 및글루코코르티코이드 유발 골다공증에서도 denosumab의 효과와 안정성이 확인되었다. 따라서, 최근의 임상 가이드라인에서 denosumab을 골다공증에 대한 1차 치료제로 권고하고있다. 이 종설에서는 denosumab의 작용기전, 약리적 특성, 유효성 및 안정성에 대해 살펴보았다.
